Redeemer's Grace Church (hereinafter referred to as "RGC") requires a virtual agreement and guardian signature on the program registration form on behalf of self and all children attending RGC Science Camp (hereinafter referred to as RGCSC) from June 20-22, 2024 and/or RGC Youth Camp (hereinafter referred to as RGCYC) from August 9-10, 2024. The camps are being run and organized by RGC, a registered Christian organization. Furthermore, this form releases RGC to photograph and/or use photographs and videos of attendees for use in its publications, advertising, promotional purposes, internet, and/or visual presentations which inform people of the services and activities of RGCSC or RGCYC. The electronic signature provided on the registration form confirms agreement to attend, participate, assumption of risk, and release of liability in order to attend RGCSC or RGCYC and to participate in any RGCSC or RGCYC activities.

In consideration of attending RGCSC or RGCYC, I acknowledge, appreciate, and agree that:
Program Risks. The purpose of the RGCSC and RGCYC programs is to teach science concepts through lessons and experiments created by our team of engineers, scientists, educators and other professionals, while introducing and spreading the Gospel and Message of the Lord Jesus Christ, and will contain elements and information regarding God, the Lord Jesus Christ and the Holy Spirit. RGC hopes to serve the community with open doors and build relationships with all local families. By my signature, I acknowledge, agree to and authorize that (1) my child(ren) will be participating in a Christian religious program, including evangelical messages, (2) my child(ren) may be photographed, or recorded by video or other media recordings during RGCSC or RGCYC and may be used in RGC Kids and Youth Ministries’ website and/or social media without compensation (3) my child(ren) will be required to follow certain rules and procedures during RGCSC or RGCYC intended for the safety, well-being and enjoyment of all participants, and (4) I or my child(ren) may receive written or email communications from the church. If any medical treatment is needed, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I and my children should carry our own health insurance.

Assumption of Risk. I am the parent or legal guardian of the child(ren) registered. I understand the legal consequences of signing this document. I, for myself and registered children, on behalf of our heirs, assigns, personal representative and next of kin, hereby release and hold harmless RGC, their officers, officials, agents, and/or employees, with respect to any and all injury, disability, death, or loss of damage to person or property, whether caused by the negligence of the releasee or otherwise, except that which is the result of gross negligence and/or wonton misconduct. I assume all related risks, both known or unknown to me, of my participation in RGCSC or RGCYC, including travel to, from and during the RGCSC or RGCYC. I understand that I am responsible for the obligations and acts of the child(ren) registered for RGCSC or RGCYC. I agree to be bound by the terms of this document.

COVID-19 Risks. RGC will be taking various precautions in light of COVID-19 to limit unnecessary contact between children and volunteers. While preventative measures will be taken to individualize activities, RGC cannot guarantee that you and/or your family will not become infected. By signing below, I acknowledge the risks of exposure may lead to property damage, personal injury, illness, permanent disability, and death. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any of the forementioned risks and consequences to my family and/or myself, whether due to COVID-19 or participation in the RGCSC or RGCYC activities. I hereby release, covenant not to sue, discharge, and hold harmless RGC and its volunteers from all liabilities, claims, actions, damages, costs, or expenses relating thereto.

Privacy. Children's age, birthday, and other personal identifying information will not be shared publicly, but only for RGCSC or RGCYC planning and organization purposes.

Binding Agreement and Severability. I understand and agree that this Release of Liability Agreement covers attendance and each and every activity and event in which I or my registered children participate hereafter, and is legally binding. This release of liability, once signed, remains in effect until withdrawn in writing emailed to Redeemer's Grace Church at children@rgcsd.com.

Signature. I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement. I understand my electronic agreement on the online registration form is legally binding.
